The effectiveness of Acceptance and Commitment Therapy on Cognitive Emotion Regulation and Distress Tolerance in Women with Obsessive-Compulsive Disorder

Introduction: Obsessive-compulsive disorder is a psychological problem that affects the quality of life, especially in women. This disorder is associated with dysfunctional patterns of cognitive emotion regulation and reduced distress tolerance. Acceptance and commitment therapy increases psychological flexibility rather than controlling symptoms. Therefore, the present study aimed to investigate the effectiveness of acceptance and commitment therapy on cognitive emotion regulation and distress tolerance in women with obsessive-compulsive disorder.
Methods: The present study was a quasi-experimental study with a pre-test-post-test design with a three-month follow-up period and a control group. The statistical population of this study included all women with obsessive-compulsive disorder referring to the Shokoeh Clinic in Isfahan in the first half of 2025, of which 30 were selected purposively and randomly assigned to an experimental group and a control group. The measurement tools included the Cognitive Emotion Regulation Questionnaire by Garnevsky et al. (2001) and the Distress Tolerance Questionnaire by Simmons and Gaher (2003). The experimental group received acceptance and commitment therapy training for eight sessions, one session per week for 90 minutes, but the control group did not receive any specific intervention. The analysis of variance with repeated measures and Bonferroni post hoc test were used to analyze the data with SPSS version 26 software.
Results: The findings showed that acceptance and commitment therapy significantly increased adaptive cognitive regulation and distress tolerance in the experimental group compared to the control group at post-test (P<0.01). The effect of this treatment remained until the follow-up stage, indicating the stability of this type of treatment.
Conclusions: These findings indicate that acceptance and commitment therapy, by strengthening psychological flexibility and promoting acceptance of internal experiences, leads to improved emotion regulation strategies and increased distress tolerance in women with obsessive-compulsive disorder and can be used as an effective method in the treatment of this disorder.
